online Athletic clearance application
ALL students interested in participating in athletics at Notre Dame High School must have a completed sports physical exam and online athletic clearance at the start of every school year. See below for the steps on how to complete the online clearance process for returning and new students.

Step 1: Go to homecampus.com

Click “For Parent & Student”; Select “California”

Return Users: Log into an existing account used in previous school year. If you have forgotten your username and/or password please email Ms. T at athompson@ndhsriverside.org for assistance.

New Users: Create an account. Please register with a valid email address as the username and generate a password.

Step 2: Select “Start Clearance Here”

Type in “Notre Dame/Riverside” & Confirm School Address

Select School Year

Add Sports - if participating in multiple sports, use “Add New Sport”

Step 3: Complete ALL Required Fields

Student Info & Parent Guardian Info

Electronic Signature Forms (Be sure to type in the EXACT match of what is on the Student & Parent/Guardian page

Download, Sign, Upload required Files

Step 4: Click “Submit Completed Application”

Step 5: Confirmation Message - Print & Sign confirmation page; submit in person or via email to Ms. T at athompson@ndhsriverside.org.
Student/Athletes are not cleared YET! Application will be reviewed. An email notification will be sent once the application has been reviewed either clearing or denying applications. Denied applications usually result in incomplete applications.
NDHS requires that all completed physicals (signed and stamped by a physician) be uploaded to your application.
