Thank you for your interest in Notre Dame High School. We ask that you provide the following documents in order to consider your application for enrollment at Notre Dame High School:

  • Transcript (Transcribed)

  • International Student Application (Form below)

  • Parent Letter (Parent giving permission to a guardian to care for student)

  • Guardian Letter (Guardian stating responsibility for the student. Must include address & notarized letter)

  • Student Letter (Must be addressed to the principal, handwritten and include DOB) · Passport/Visa(ColorCopy)

  • Grade Verification Letter

  • Immunization Records/TBTest (ResultsReported)/TDAP

  • Birth Certificate

  • Bank Statement/Tuition Agreement/Certificate of Deposit

Paperwork will be sent to the Diocese of San Bernardino to process the student’s I-20 only after all of the above has been completed.

Once the application is completed and reviewed, the initial zoom interview with the student will be scheduled.
